Consent to AI-Assisted Documentation
Informed consent and notice for the use of ABAIQ, an artificial-intelligence documentation assistant, in the client's ABA services.

1. Why we are asking
Our clinicians (Board Certified Behavior Analysts, assistant behavior analysts and behavior technicians) use a software tool called ABAIQ, made by Hybreu Digital LLC, to help write and review the clinical documentation of each session. ABAIQ uses artificial intelligence (AI). Professional standards for ABA, including the Practice Parameters for Artificial Intelligence Use in Applied Behavior Analysis of the Council of Autism Service Providers (CASP) and the BACB Ethics Code, ask us to tell you how AI is used, its benefits and risks, and to give you the choice to decline. This form does that.
2. What ABAIQ does, and what it does not do
It helps write. After a session, the clinician enters what happened (the activities, the behaviors observed, the strategies used, how your child responded). ABAIQ turns that information into a draft note: session notes, supervision notes, caregiver training notes, protocol modification notes and monthly progress summaries. It can also read the treatment plan the clinician uploads so the draft uses your child's own goals and strategies, and it can draw graphs from the data the clinician enters.
It helps review. With the ABAIQ QA Suite, the technician's note is sent to the supervising behavior analyst, who receives an AI-assisted documentation-quality check (is the note complete, does it match the treatment plan) and reviews the note before it is finalized.
It does not treat, decide or diagnose. ABAIQ never interacts with your child. It does not choose treatment, change the plan, make clinical or diagnostic decisions, or decide anything about insurance. Every note is a draft that the clinician reads, corrects where needed, and signs. The clinician remains fully responsible for the documentation and for every clinical decision.
3. What information is used and who can see it
- The information: your child's name or initials, date of service, the treatment plan (target behaviors, their functions, the strategies and goals), and the clinician's description of the session. Only what is needed for the note is used.
- Our practice keeps your child's official record in its own system. ABAIQ is not that record.
- Hybreu Digital LLC (ABAIQ) processes the information as our "business associate" under HIPAA, bound by a written Business Associate Agreement. Its servers are HIPAA-eligible, located in the United States, and the information is encrypted in transit and at rest.
- ABAIQ's technology providers (its cloud host and its AI providers) are also bound by HIPAA business associate agreements. The AI providers are configured for zero data retention: they do not store your child's information and do not use it to train their systems.
- Nobody else. Your child's information is never sold, never used for marketing, and never used to train artificial intelligence.
4. How long the information is kept
Draft notes written with the ABAIQ extension are delivered to the clinician and are not kept by ABAIQ afterward. Notes sent for supervisory review through the QA Suite, together with the AI check and the supervisor's feedback, are kept on ABAIQ's encrypted servers as part of the supervision record for the period that HIPAA and our record-keeping policy require (at least six years), and can be deleted at our request when that period ends.
5. Benefits and risks
Benefits. More complete and consistent notes, less time typing and more time with your child, and faster feedback from the supervising analyst to the technician.
Risks. Artificial intelligence can make mistakes, for example describing something inaccurately or filling a gap with wrong information. That is why every note is reviewed and signed by the clinician before it becomes part of the record. As with any digital system, there is a small risk of a data breach or technical error despite the safeguards described above; no system is perfectly secure.
6. Your rights
- You may decline now, or withdraw your consent at any time, by telling your clinician or writing to our practice. Declining or withdrawing will not affect your child's services in any way.
- If you decline, the clinician writes and reviews the documentation without AI assistance (the non-AI alternative).
- You may ask questions about how AI is used at any time, and you may request a copy of this form.
- Your right to access your child's records is exercised with our practice, which maintains the official record.
7. Your decision
